The name

“Body Protection Compound” names a larger protein reported from human gastric juice; BPC-157 is a 15-residue partial sequence of it.4 It is described as a stable gastric pentadecapeptide and is made synthetically.

Free base and acetate

FDA identifies two forms: the free base and an acetate salt with variable acetate content. It also notes that several salts and derivatives are sold under the one name.8

Endothelial signalling and angiogenesis

VEGFR2 ↑ expression, internalisation→Akt→eNOS
Src→Caveolin-1 releases eNOS→NO

Increased vessel density in chick membrane and endothelial tube assays, and faster blood-flow recovery in ischaemic rat hind limbs. VEGFR2 rose; VEGF-A did not.2 In isolated rat aorta it caused endothelium-dependent, nitric-oxide-mediated vasodilation.3

ModelsHuman endothelial cells, chick chorioallantoic membrane, rat hind-limb ischaemia, rat aorta

Cell adhesion and migration

FAK phosphorylation ↑→Paxillin phosphorylation ↑→F-actin, spreading→Migration

Faster outgrowth of fibroblasts from tendon explants, better survival under oxidative stress, and dose-dependent increases in migration and spreading. Proliferation was not directly affected.4

ModelRat Achilles tendon fibroblasts and explants

Tissue response to injury

ERK1/2→c-Fos, c-Jun, Egr-1→Granulation, collagen
Inflammatory cytokines ↓Growth hormone receptor ↑

Topical application sped closure of alkali burns in rats, with better granulation, re-epithelialisation and collagen deposition.5 Across preclinical studies, a 2025 systematic review reports lower inflammatory cytokines and higher growth hormone receptor expression.6

03 · Applications

Where the interest lies

Animal studies span the gut, liver, skin, muscle, tendon, bladder, blood vessels and nervous system.7 BPC-157 is not approved as a medicine by FDA or other regulators7 and is banned in professional sport.6

Tendon, ligament, muscle and bone

Preclinical · early human

Improved functional, structural and biomechanical outcomes across preclinical injury models.6 A placebo-controlled phase 2 trial in acute hamstring strain is recruiting10, and a pilot after rotator cuff repair is registered.11

Gastrointestinal mucosa

Preclinical · one controlled human study

Rat models include colonic fistulas and NSAID-induced lesions. In 53 patients with mild-to-moderate ulcerative colitis, a two-week enema did not differ significantly from placebo; the study appears only as an abstract.8

Wound healing

Preclinical

Topical application accelerated closure of chemical burns in rats.5

04 · Exposure

What reaches tissue, by route

Any pathway effect has to survive the journey to tissue first. In rats and dogs the intact peptide clears with a half-life under 30 minutes and is undetectable four hours after a dose; radioactivity from a labelled dose peaks in kidney, liver and stomach wall and is lowest in brain and fat (total label, not intact peptide).13 No human pharmacokinetic data exist beyond one finding: plasma levels were undetectable after rectal dosing. Below, what each route has behind it in people first, then in animals. Blank means no data in the sources cited.

RouteHuman reportsExposure and toxicityAnimal efficacy
IntramuscularNone reported.8Rats and dogs: peak plasma at 3–9 min, half-life under 30 min, bioavailability 15–19% (rat) and 45–51% (dog). 28-day toxicity studies in both species.13,8Most of the Zagreb-group models: gut, liver, wounds. The deepest animal record of any route.7,8
IntravenousTwo healthy adults, 2025 safety pilot.8Half-life 15 min (rat), 5 min (dog).13Pharmacokinetic use only.13
SubcutaneousNone reported.8No pharmacokinetic or toxicity data.8Rodent models.7
OralPhase 1 tablet study registered 2015; results never posted.8No pharmacokinetic or toxicity data.8Intragastric dosing in rodent models.7
Rectal24 healthy volunteers over 8 days; 53 ulcerative colitis patients, no significant difference from placebo.8Plasma levels undetectable after enema in volunteers.8Not reported in the sources here.
TopicalNone reported.8No pharmacokinetic or toxicity data.8Rat alkali burn; creams, gels and dressings.5,7

05 · Clinical maturity

How far the evidence goes

PreclinicalBroad and promising

35 of 36 studies in a 2025 systematic review were preclinical; the authors conclude BPC-157 shows promise for musculoskeletal recovery.6 Most studies FDA reviewed came from one Zagreb group.8

HumanSmall and exploratory

Five reports of 2 to 53 people. FDA describes them as short, small and exploratory; no serious adverse events were reported.8

AnecdotalGrowing use

Increasingly used by clinicians and athletes.6 In a phone survey, 11 of 12 knee-pain patients reported significant improvement after injection, without validated measures or a control group.9

RegulatoryUnder review

In July 2026 an FDA advisory committee voted 8 to 6 to allow compounding for ulcerative colitis, against FDA staff advice. The vote is not binding.12

06 · Takeaway
Interest in BPC-157 comes from reported effects across several repair-related pathways. The unresolved question is how reliably those experimental findings translate into meaningful outcomes in humans.
